I have a contact that does this for a living. Not sure what he'd charge but I am sure he'd be reasonably fair. You can see some of his work he did for me on the "thunder" shirts from this last year.

I use Vistaprint, They've done some great printing for me. My T-shirts and shop banner came out great.

Local Kinkos does a great job as well. Local, decent prices and usually do it while you wait or take a couple hours.
